Card of insurance

New regulations introduced recently require that citizens of the nationalities listed below fill in the 'card of insurance' for their visa application to show they have the necessary travel and medical insurance cover.

If you are a citizen of a Shengen State - Austria,  Belgium, France, Germany, Greece, Holland, Iceland, Italy, Luxembourg, Norway, Portugal, Spain, Sweden, or Czech Republic, Estonia, Finland and Israel, print out this page, cut out the form, fill in the details and include it with your application.

Failure to provide correct details of your insurance cover could lead to the refusal of your visa application.
